We will need to receive a copy of your referral from either your GP or Optometrist before making an appointment.

First Visit

It would be preferable if you could arrive 10 minutes prior to your consultation to register.

What do I need to bring to my appointment?

  • Details of third parties who will receive copies of correspondence
  • Signed consent to collect personal information.
  • Relevant Tests and Correspondence.
  • Lists of any Medications you are currently taking.
  • Any Reading or Long Distance Glasses you are currently wearing.
  • Medicare/DVA card
  • Pension or Health Care Card
  • Private Health fund details
